[PRL] Availability of Wadsworth's thesis

While at Tech, I tried to get the following dissertation through ILL, 
but unfortunately the microfiche that came from overseas was not the 
thesis which I had requested and the correct one had not been sent by 
the time I left so I never could make a copy of it for my own use.  
Does anyone have it in any form?
